Our Programs
T4 Research Program
This program serves as the core opportunity to support T4 translation research in heart, lung, blood, and sleep disorders, with implementation research outcomes as primary endpoints. This program will advance our understanding of implementation strategies across diverse patient populations and health systems. It also will significantly impact care of heart, lung, blood, and sleep disorders by decreasing the ‘gap in translation’ from clinical trials and academic centers to communities where most health services are delivered.

Small Business Topic of Special Interest Program
This program leverages the Institute’s Small Business grant mechanisms to support the development of innovative technology, service delivery models, or designs to increase the adoption, uptake, and sustainability of evidence-based guideline recommendations for the management of heart, lung, blood, and sleep disorders. These technologies or service delivery models should include multi-level (health systems, provider, and patient) facets and benefit certain populations, such as racial or ethnic minority groups, rural populations, and low socioeconomic status groups.
